In the OOTB approval workflow, if “Cancel the workflow when the document is changed/rejected” is enabled,this error may occur. This applies to SharePoint2007.
This is because if you also use the workflow to change the approval status of document, when the document approval status is udpated, it is considered a “Change” to the document and it triggers the workflow to be canceled.This is really a bug and I hope it gets fixed in the next version.
